Platinum Healthcare Services (PHS) is a DBHDS-licensed provider offering group home residential services for adults with intellectual and developmental disabilities. We believe that all individuals can be contributing and valued members of their communities, and we partner closely with residents, families, and support coordinators to help them achieve meaningful goals.

Personalized Care
Daily living assistance (personal care, hygiene, meals, and household routines)
Medication management and coordination with healthcare providers
Skilled Nursing
Behavior and emotional support strategies individualized to each resident
Skill-building, habilitation, and community integration opportunities

Recreational and therapeutic activities that encourage social engagement
Transportation to appointments, community programs, and day activities
Referrals / Admissions
Who We Serve
Adults (ages 18 and older) with intellectual and developmental disabilities who are eligible for services under the Community Living (CL) or Building Independence (BI) Waivers.
Required Documentation
Current ISP (Parts I–V, including all protocols)
Comprehensive documentation outlining each resident’s individualized support plan, goals, and protocols to ensure consistent, person-centered care and progress monitoring.
Quarterly Reports
Detailed summaries completed every three months to review each resident’s progress, update goals, and ensure services remain aligned with their individualized support plan.
SIS, RAT, MAR, and VIDES/LOF
Assessment and documentation tools used to evaluate support needs, track medication administration, monitor progress, and maintain accurate records of each resident’s care and development.
Psychological Evaluation
A comprehensive assessment conducted by a licensed professional to understand a resident’s cognitive, emotional, and behavioral functioning, guiding individualized support and treatment planning.
Current Physical and recent medical history
Up-to-date health information and physical examination results documenting the resident’s medical status, recent treatments, and any ongoing health concerns to support appropriate care planning.
